29 CFR 1910.23(e)(4): A standard toeboard was not 4 inches nominal in vertical height from its top edge to the level of the floor, platform, runway, or ramp:   (a) Warehouse Mezzanine: On or about 12 June 2013 and at times prior a gated section of the handrail system on the mezzanine did not have a toeboard attached exposing employees on the floor below to the hazard of falling materials.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:    (a)  Warehouse: On or about 12 June 2013 at times prior employees were exposed to entrapment hazards in the event of a fire. There were no signs posted along the exit access route when the exit discharge was blocked by inventory.

29 CFR 1910.304(f)(1)(iv):   Overcurrent devices for circuits rated 600 volts, nominal, or less, were not readily accessible to each employee or authorized building management personnel:  (a) Warehouse: On or about 12 June 2013 and at times prior, employees were exposed to trips, falls and the hazards associated with being unable to de-energize equipment in the event of an emergency. The breaker panel in the beer product recycle room was blocked by storage.
