29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    a.  On or about September 10, 2013, at the above addressed jobsite, the employer failed to establish and follow specific energy control procedures in accordance with manufacturer's developed guidelines, to ensure the securing/blocking of the powered industrial truck's fork carriage/mast when working under an elevated mast.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:    a.  On or about September 10, 2013, at the above addressed jobsite, the employer failed to provide training in the procedures for controlling hazardous energy sources as applicable to servicing roof exhaust fans for employees authorized to perform such work, exposing employees to amputation hazards.        Publix Supermarkets Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315479907, citation number 1, item number 1 and was affirmed as a final order on September 17, 2012, with respect to a workplace located at 9786 W Beaver Street, Jacksonville, FL.
