29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At Lot 118 Old Field Subdivision Daphne, Alabama:  On or about June 14, 2013 and at times prior; the employer failed to provide fall protection for his employees involved in roofing activities on a high slope roof exposing them to fall hazards of 9.4 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)  At Lot 118 Old Field Subdivision Daphne, Alabama:  On or about June 14, 2013 and at times prior; the employer failed to train each employee on the hazards of falls to include but not limited to the nature of fall hazards, the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection used exposing his employees to fall hazards of 9.4 feet to the ground below.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:  a) At Lot 118 Old Field Subdivision Daphne, Alabama:  On or about June 14, 2013 and at times prior; the employer allowed his employees to used a portable A-frame ladder leaned against the house to gain access to and from the roof exposing his employees to fall hazards of 9.4 feet to the ground below.
