29 CFR 1926.152(a)(1): Containers other than approved metal safety cans or DOT approved containers were used for the handling and use of flammable liquids in quantities of 5 gallons or less:   On or about 06/14/13 at Stockbridge, GA:  Employees were exposed to explosive / fire hazards while using two plastic fuel containers used to refuel equipment to include a Dewalt Model D55270 air compressor.  The employer failed provide employees an approved metal safety can for use when refueling equipment at a work site.

29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:   On or about 06/14/13 at Stockbridge, GA: Employees were exposed to struck-by or caught between hazards while operating a Dewalt D55270 air compressor, which was missing the cover over the rope pull type starting mechanism.  The employer failed to protect employees by ensuring all exposed moving parts were properly guarded.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section:  On or about 06/14/13 at Stockbridge, GA:  An employee was exposed to struck-by injury when performing work at ground level, while roofers were removing existing roof tiles to the ground below.  The employer failed to provide a hard hat to the employee who was exposed to the falling roofing tiles and debris.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  On or about 06/14/13 at Stockbridge, GA: Employees were exposed to fall hazards while performing roofing work, and using two portable extension ladders to access the roof of a residential home.  The employer failed to ensure that the ladders' side rails extended three feet above the landing surface of the roof.
