The employer failed provide employees an approved metal safety can for use when refueling equipment at a work site.

The employer failed to provide a hard hat to the employee who was exposed to the falling roofing tiles and debris.

On or about 06/14/13 at Stockbridge, GA: Employees were exposed to fall hazards while performing roofing work, and using two portable extension ladders to access the roof of a residential home.  The employer failed to ensure that the ladders' side rails extended three feet above the landing surface of the roof.
