29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented.                                                                                                              16603 Riggs St., Omaha, NE 68105 (Job-site): The employer did not ensure that adequate fall protection was provided for employees who were working on the roof of a home which was approximately 11 feet from a lower level.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.

The portable ladder being used by employees to access the roof did not extend at least 3 feet above the roof.    Abatement certification is required for this violation. The ladder was being used at height of the top support of approximately 11 feet high and 7 feet distance between the foot and top support).    Abatement certification is required for this violation. Employees were using a portable ladder to access an upper landing surface.  The side rails did not extend at least 3 feet above the upper landing surface and the ladder was not set up at an angle of approximately one-quarter the working length of the ladder. The ladder was being used at height of the top support of approximately 11 feet high and 7 feet distance between the foot and top support).    Abatement certification is required for this violation.
