Bennett's Valley Ambulance Association- The written Bloodborne Pathogen Exposure Control Plan, provided on or about June 14, 2013, did not include the exposure determination.  Company employees were exposed to blood or other potentially infectious material intermittently throughout each work shift, when for instance, treating wounds, performing CPR or cleaning blood or other potentially infectious material (OPIM) spills.

29 CFR 1910.1030(c)(1)(ii)(B): The schedule and method of implementation for paragraphs (d) Methods of Compliance, (e) HIV and HBV Research Laboratories and Production Facilities, (f) Hepatitis B Vaccination and Post-Exposure Evaluation and Follow-up, (g) Communication of Hazards to Employees, and (h) Recordkeeping, of this standard, and   a.)Bennett's Valley Ambulance Association - For example, the written Bloodborne Pathogen Exposure Control Plan, provided on  June 14, 2013, did not include the schedule and/or method of implementation for the following requirement paragraphs in 29 CFR 1910.1030:       1.) 1910.1030(f)(1)(ii)(C): The employer shall ensure that post-exposure evaluation and follow-up procedures, including prophylaxis, are performed by or under the supervision of a licensed physician or by or under the supervision of another licensed healthcare professional:    The plan stated that medical evaluations would be performed, no methods of implementation (who would be performing the evaluations) were included.    2.) 1910.1030(g)(2)(i): Employers shall ensure that all employees with occupational exposure participate in a training program:    The plan stated that training would be done, no method of implementation (how would the training be done and who would do it) was included.  3.) 1910.1030(h)(1): The employer shall establish and maintain an accurate record for each employee with occupational exposure, in accordance with 29 CFR 1910.1020:    The plan stated that medical records would be kept confidential, no methods of implementation (who would be responsible for the records and where the records would be kept) were included.

Bennett's Valley Ambulance Association- On or about June 14, 2013, and times prior thereto: Company employees were exposed to blood or other potentially infectious material intermittently throughout each work shift, when for instance, treating wounds, performing CPR or cleaning blood or other potentially infectious material (OPIM) spills. Company employees had not received annual training on what Bloodborne pathogens were and how to properly protect themselves from exposure to blood or OPIM. Company was unable to affirm when training was last received.

Bennett's Valley Ambulance Association- On or about June 14, 2013, the employer had not obtained records of employees' hepatitis B vaccination records. The employer was unable to provide such documents upon request.
