29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     The employer failed to protect employees from potential electrical, caught between and amputation hazards associated with performing maintenance on equipment requiring the de-energizing of stored energy without having developed and implemented a written lockout and tag out program.  This was most recently evident during the inspection of the facility located at 2036 Kilauea Ave, Hilo, HI 96720, where the employer has employees operating and maintaining equipment that requires lockout and/or tag out as the means of securing the de-energizing of the equipment but the employer has not developed a written lockout or tag out program to address how they will ensure employees are protected against accidental exposure.  Instances include but are not limited to:     i.  Employees operating and maintaining the custom noodle machine which is hard wired into a blade switch as its sole source of power but employer does not apply locks nor has the employer trained any employees to recognize the hazards of personnel working on electrical equipment without having a lock to secure that the equipment is kept off.   ii. Employees operating and maintaining the custom mixer which is simultaneously wired into the same blade switch at the noodle machine, again without having trained the employees on the hazards or techniques to prevent accidental startup.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    The employer failed to protect employees from potential amputation hazards associated with operating and working near unguarded conveyor belt system.  This was most recently evident during the inspection of the facility located at 2036 Kilauea Ave, Hilo, HI 96720, where the employer has employees placing unsealed bags of product on the sealer convery belt and where the employee stands there is no guarding over the nip point created by the horizontal conveyor belt and pulley.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     The employer failed to protect employees from potential chemical hazards associated with using cleaners and other chemicals in the work place without having developed a written program that addresses the methods for which they will be used.  This was most recently evident during the inspection of the facility located at 2036 Kilauea Ave, Hilo, HI 96720, where the employer has employees cleaning the facility on a daily basis using some basic house hold cleaners without having developed a written program that addresses their use, storage or training of employees.  Chemicals include but are not limited to: Clorox bleach, WD-40, Renuzit home fragrance, Ajax cleaner, ABC dry chemical and Dawn liquid dish soap.
