29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    a) 4901 Union Deposit Road, Harrisburg Pa - An employee was observed installing a rigger for a roof awning while working from a metal deck and was not provided fall protection, exposed to a fall hazard from 18 feet - 4 inches up to 20 feet - 7 inches, on or about June 17, 2013.    No abatement certification or documentation required.

29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:    a) 4901 Union Deposit Road, Harrisburg Pa - An employee working from a Genie S-60X aerial lift was observed standing on the rails of the basket and was exposed to an approximate 18 foot fall hazard to the ground below, on or about June 17, 2013.      b) 4901 Union Deposit Road, Harrisburg Pa - An employee was observed exiting a roof and entering the basket of a Genie S-60X aerial lift by climbing the rails of the basket and was exposed to an 18 foot - 4 inch fall hazard, on or about June 17, 2013.    K-O Construction Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.453(b)(2)(iv) which was contained in OSHA inspection number 721384, Citation number 1, Item number 2, and was affirmed as a final order on December 3, 2012, with respect to a workplace located at 1158 River Road, Marietta, PA 17547.    No abatement certification or documentation required.
