29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:     (a) On or about June 18, 2013, in the laundry room, the two carbon dioxide cylinders used for carbonation of the soda machine were not secured with a chain to prevent them from being toppled over.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:       (a) On or about June 18, 2013, in the laundry room, a power strip was plugged into an extension that was powering a Lasko portable fan.     (b) On or about June 18, 2013, in the laundry room, an extension cord had been wired into the wall mounted exhaust fan that was plugged into a power strip.     (c) On or about June 18, 2013, in the laundry room, the pump for the soda machine was being powered with a non-insulated extension cord.

The employer did not develop a written hazard communication program which includes an inventory of the chemicals present at the site as required by 1910.1200(e)(1)(i).
