29 CFR 1910.106(g)(3)(iv)(d): The dispensing units, except those attached to containers, shall be mounted either on a concrete island or protected against collision damage by suitable means.  Refueling area; on or about 11 June 2013 and times prior thereto; above ground fuel storage tanks were not guarded to prevent vehicular contact. This exposed employees to contact with flammable liquids and other fire hazards.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch.  Maintenance shop; on or about 11 June 2013 and times prior thereto; the work rest on a bench grinder was not properly adjusted to a maximum of 1/8 of an inch from the abrasive wheel to prevent the work from being jammed between the abrasive wheel and the work rest. The gap between the abrasive wheel and work rest measured approximately 7/16 of an inch. This condition exposed employees to a caught in hazard.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective.  Maintenance shop; on or about 11 June 2013 and times prior thereto; a 50 foot heavy duty magenta extension cord had a ground pin missing. This condition exposed employees to an electrical shock and burn hazard.
