This condition exposed employees to an electrical shock and burn hazard.

Bus shop, Bays 3, 4, and 5; on or about 11 July 2016 and times prior thereto; three instances were noted where extension cords were used as a substitute for permanent wring to power the garage roll-up doors. a. Bay 3; A 25-foot blue extension cord was used as permanent wiring to power the roll-up door. b. Bay 4; A 25-foot beige extension cord was used as permanent wiring to power the roll-up door. c. Bay 5; A 25-foot blue extension cord was used as permanent wiring to power the roll-up door. This condition exposed employees to electrical shock and electrical burn hazard.
