29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times.  Vehicle Maintenance Building, 2nd floor, Human Resource office:  On or about 11 June 2013 and times prior thereto, the emergency exit light were inoperative.  This condition exposed employees to delays in exiting the building in the event of smoke and fire hazards.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  Old Maintenance shop area, south wall, inside flammable locker, top shelf:  On or about  11 June 2013 and times prior thereto, an unattended plastic bottle; containing an unknown chemical substance was not marked identifying the product and the hazard warnings associated with it.  This condition exposed employees to chemical burns, poisoning, and skin dermatitis.
