29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:     a) On or about June 17, 2013, the employer did not ensure that employees were using proper personal protective equipment.  Employees were installing new roof shingles using nail guns without using adequate eye protection.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of 29 CFR 1926.501.:       On or about June 17, 2013, employees were exposed to fall hazards in excess of 20 feet, while installing a new roof without adequate fall protection.  The employer did not enforce the use of a fall restraint or an arrest system nor did the employer implement an alternative fall protection plan to address the fall hazards in the following locations:           a) Building #11 had workers that were wearing safety harnesses but the harnesses were not anchored or attached to either a rope grab system, lanyard, lifeline or to an anchor system.         b) Building #10 had workers that were not wearing any safety harnesses, lanyard, or lifeline to be used for fall protection.
