While the cover is not in place, the pit or trap opening was not constantly attended by someone or was not protected on all exposed sides by removable standard railings: a) Planer area floor: On June 12, 2013 and at times prior, employees were exposed to tripping and fall hazards while working/walking in and around an unguarded, 18 inch by 18 inch, floor opening.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour: a) Outside break room: On June 12, 2013 and at times prior, oxygen cylinders were stored with a fuel-gas cylinder (acetylene) outside the employee break room. Employees were exposed to burns from fires that would be accelerated by the oxygen and fuel-gas combination.
