Worksite: The employer did not assess the work0place to determine not only the hazards associated with walking and working in and around the skylights on the roof, but also appropriate types of fall protection systems and/or devices.     On or about 6/19/2013, an employee walking on the above roof fell approximately 17 feet to the ground throuigh a skylight and sustained severe injuries as a result.

Worksite: The employer did not ensure through providing specific instruction and/or other means that the employee who was required to walk across the roof was protected from falling through the skylights.   On or about 6/19/2013 an employee, walking on the roof fell approximately 17 feet to the ground through one of skylights and sustained severe injuries as a result.
