29 CFR 1910.334(a)(3)(i): A flexible cord used with grounding type equipment did not contain an equipment grounding conductor; a. On or about June 17, 2013 and at times prior, an employee was exposed to electric shock injuries while using a floor stripper. The stripper cord set was missing a ground pin and was used by the employee while working on a wet floor.
