29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. Instance a.) On or about May 31, 2013 and times prior in the 800 Batching, several large containers used in chemical mixing and vacuum machines used for cleanup had chemical residues built up exposing employees to chemical hazards from inhalation and physical contact. Instance b.) On or about May 31, 2013 and times prior in the 640 Washcoat Area, baghouse filter waste collection equipment was leaking fugitive dust into the area exposing employees to chemical inhalation and physical contact hazards.

1910.37 (a)(4) Safeguards designed to protect employees during an emergency (i.e. exit lighting) were not in proper working order at all times. On or about May 31, 2013 and times prior to, an Emergency EXIT sign located in the 800 Batching Area did not illuminate, exposing employees to hazards from delayed urgent egress.
