29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided when necessary whenever hazards capable of causing injury and impairment were encountered: a) In the clinic, on or about June 11, 2013, and at times prior thereto, employees administered Eligard (Leuprolide Acetate), an antineoplastic agent, without the use of chemotherapy gloves, disposable nonlinting and nonabsorbent gown and goggles. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312394661, Notice number 2, Item number 1 and was affirmed as a final order on 10/19/2011, with respect to a workplace located at the Grants Pass Outpatient Clinic.

29 CFR 1910.1030(g)(1)(i)(A): Warning labels were not affixed to refrigerators, or freezers containing blood or other potentially infectious material: a) In the laboratory the refrigerator and freezer used to store blood samples did not have biohazard labels posted to communicate the hazard to employees. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312392285, Notice number 1, Item number 6 and was affirmed as a final order on 11/9/2010, with respect to a workplace located at the Veterans Affairs Medical Center, Portland.
