29 CFR 1910.1030(g)(2)(vii)(F): The bloodborne pathogens training program did not contain an explanation of the use or limitations of methods that would prevent or reduce exposure including appropriate engineering controls, work practices or personal protective equipment: a) Establishment - Employees who were required to use sharps including, but not limited to needles used for the administration of Lovenox, were not trained in the proper use of the respective engineering controls (safety devices) in a manner meeting the requirements of the standard. On or about 6/18/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1030(c)(1)(ii): The employer's Exposure Control Plan did not contain at least the elements in 29 CFR 1910.1030(c)(1)(ii)(A) through (c)(1)(ii)(C): a) Establishment - The employers written Bloodborne Pathogens Exposure Control Plan did not include all of the elements required by the standard including, but not limited to an employee exposure determination, specifying the medical facilities employees would be sent to in the event of an exposure incident, the requirement for receiving a physicians written opinion after an exposure incident, specifying a cleaning/disinfecting schedule and identifying the disinfecting products to be used, and the identification of engineering controls for sharps and the training requirements for those employees required to use them. On or about 6/18/13. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
