29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    On or about June 18, 2013, the employer did not ensure each employee engaged in residential construction activities was protected from falling. An employee performing roofing work at a height of over 6 feet was exposed to an unprotected fall of approximately 10 feet to concrete below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards.   On or about June 19, 2013, the employer did not ensure each employee engaged in roofing work was trained to recognize and avoid fall hazards and the procedures to follow to minimize or avoid those hazards.
