On or about June 9, 2013, at the job site located at 1001 Rochier Ave in Fayetteville Arkansas, the employer directed employees to operate aerial lifts who were not qualified to do so. As a result the employees operated the aerial lift on unequal ground and elevated more than 40 feet near energized power lines while exposed to fall and electric shock hazards.
