29 CFR 1910.178(l)(3)(ii)(I):     Powered industrial truck operators did not receive initial training on other unique or potetially hazardous environmental conditions in the workplace that could affect safe operation.    Employees were exposed to hazards when loading and unloading trailers because the employer did not provide training on a means or procedure that would secure the trailer to the loading dock. One or about June 13, 2013 a tractor trailer pulled away from the loading dock when a forklift was still inside the trailer.
