29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Construction Site; On or about June 19, 2013 and at times prior, as the controlling employer, Jackson Contractor Group, Inc. did not ensure the employees of its sub-contractor, Just Rite Acoustics were protected from tripping hazards due to the approximate 8 inch diameter holes in the Tate Access Flooring system they were installing.
