29 CFR 1926.21(b)(1):   An accident prevention program had not been initiated by the employer:   At the job site employee(s) engaged in trenching activities were exposed to physical injuries due to no safety and health programs being developed and implemented to address the steps necessary to eliminate or reduce construction hazards such as, but not limited to the following:  working with ladders, laying pipe in trenches, falls, electrical safety, and etc.

29 CFR 1926.602(a)(9)(i): Bidirectional machine(s) were not equipped with an operable horn, distinguishable from the surrounding noise level, which could be operated as needed when the machine is moving in either direction:       (a)  Employee(s) operating the Komatsu Loader (#44) at the job site were exposing employee(s) walking around the job site to a crushing hazard due to no functional horn being available to warn anyone from inadvertent contact with the Komatsu Loader (#44).    (b)  Employee(s) operating the Bobcat T300 (#64) at the job site were exposing employee(s) walking around the job site to a crushing hazard due to no functional horn being available to warn anyone from inadvertent contact with the Bobcat T300 (#64).
