OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard:  On or about June 19, 2013, at the above addressed site, an employee was exposed to a potential fall from being raised in a handmade wooden box supported by the forks of a Toyota forklift.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On or about June 19, 2013, at the above addressed site, employees were exposed to potential electrical shock and or burns due to a wall mounted electrical panel not being provided with sufficient access while being blocked by cardboard boxes stored on a wooden pallet directly in front of that panel.    VERTILUX LIMITED WAS PREVIOSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.303(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 896220, CITATION NUMBER 1, ITEM NUMBER 3, AND WAS AFFIRMED AS FINAL ORDER ON 05/29/2013, WITH RESPECT TO A WORKPLACE LOCATED AT 7300 NW 35th TERRACE IN MIAMI, FLORIDA.

29 CFR 1910.37(b)(5): Each doorway or passage along an exit access that could be mistaken for an exit was not marked "Not an Exit" or similar designation, or be identified by a sign indicating its actual use (e.g. closet):  a) On or about June 19, 2013, at the above addressed site, employees were exposed to being trapped due to doorway in the supervisors/shipping office not being marked "Not an Exit."  b) On or about June 19, 2013, at the above addressed site, employees were exposed to being trapped due to doorway on the loading dock in the southeast corner of the warehouse not being marked "Not an Exit."
