29 CFR 1926.502(b)(2)(i): Midrails were not installed at a height midway between the top edge of the guardrail system and the walking/working level: Show room mezzanine: On or about June 12, 2013, employees preparing to construct drywalls on the mezzanine were along a section of the mezzanine open side that did not have a midrail in its guardrail system. One employee was kneeling and was below the top rail. This exposed the employees to the hazard of falling over 6 feet.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge: Showroom mezzanine stairway: On or about June 12, 2013, employees preparing to construct the mezzanine drywalls used the mezzanine stairway. The stairway did not have handrails or stairrails on its open sides exposing the employees to the hazard of falling.
