29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 19, 2013, at the above addressed jobsite, an employee was engaged in painting work from a small rental community roof with a slope of 4:12 at approximately 10 feet above ground level without a means of fall protection, exposing him to a fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about June 19, 2013, at the above addressed jobsite, an employee was using ladder that did not extend at least 3 feet above the upper level or landing of a small rental community roof with a slope of 4:12 at approximately 10 feet above ground level, exposing the employee to a fall hazard.
