29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    The employer does not develop, document and utilize procedures for the control of potentially hazardous energy to protect employees from injury.       a. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Streamline waterjet cutter, Model# SL-V 60 Plus, S/N 0602064 without written procedures for the control of the release of potentially hazardous 440VAC electrical, pneumatic and thermal energy.       b. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Adira Shear, Model# GHS-630, S/N 4116/1076D without written procedures for the control of the release of potentially hazardous 440VAC electrical and hydraulic energy.    c. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Adira Press Brake, Model# QHD 13540, S/N 5727/10642 without written procedures for the control of the release of potentially hazardous 440VAC electrical and hydraulic energy.      d. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Adira Press Brake, Model# QHD 5020, S/N 5630/10543 without written procedures for the control of the release of potentially hazardous 440VAC electrical and hydraulic energy.      e. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Adira Press Brake, Model# QHD 11030, S/N 7019/12075 without written procedures for the control of the release of potentially hazardous 440VAC electrical and hydraulic energy.          f. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a RoboVent Plasma Laser, Model# PAL-25M-5000-4, S/N 22054 without written procedures for the control of the release of potentially hazardous 480VAC electrical and pneumatic energy.      g. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on a Champion Air Compressor, S/N D081351 without written procedures for the control of the release of potentially hazardous electrical and pneumatic energy.       h. This violation occurred on or about June 18, 2013, in the production area where employees were exposed to electrical, caught-in, and struck-by hazards when performing set-up and service on an Ingersol-Rand Air Compressor without written procedures for the control of the release of potentially hazardous electrical and pneumatic energy.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that procedures are developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in activities covered by this standard.

This violation occurred on or about June 18, 2013, in the center of the facility where an employee was exposed to a stuck-by hazard when operating an Adira shear, Model #GHS-630, S/N 4116/1076D, without a point of operation guard.    b. This violation occurred on or about June 18, 2013, in the rear of the facility where an employee was exposed to a stuck-by hazard when operating a Pemserter , Model #2000, without a point of operation guard.    c. This violation occurred on or about June 18, 2013, in the rear of the facility where an employee was exposed to a stuck-by hazard when operating a Strippit Punch, Model #Super 30-40HD, without a point of operation guard.    d. This violation occurred on or about June 18, 2013, in the rear of the facility where an employee was exposed to a stuck-by hazard when operating an Adira press brake, Model #GHD-13540, S/N 5727/10642, without a point of operation guard.    e. This violation occurred on or about June 18, 2013, in the rear of the facility where an employee was exposed to a stuck-by hazard when operating an Adira press brake, Model #GHD-5020, S/N 5630/10453, without a point of operation guard.    f. This violation occurred on or about June 18, 2013, in the rear of the facility where an employee was exposed to a stuck-by hazard when operating an Adira press brake, Model #GHD-11030PLS, S/N 7019/12075, without a point of operation guard.    g. This violation occurred on or about June 18, 2013, in the center of the facility where an employee was exposed to a struck-by hazard when operating a Kalamazoo band saw, Model #H9AW, without a point of operation guard.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that guards are installed on all machines to protect employees from the point of operation.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    The employer does not effectively close unused openings on electrical boxes that can expose employees to injury. This violation was observed on or about June 18, 2013, in the center of the facility where employees were exposed to a 440VAC electrical shock hazard when working around a junction box with a knockout opening that was not effectively closed.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that unused openings in boxes, cabinets, or fittings are effectively closed.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    The employer does provide covers for electrical junction boxes. This violation was observed on or about June 18, 2013, in the rear of the facility where employees were exposed to a 250 VAC electrical shock hazard when operating a Boshert Lite hydraulic notcher without a cover on the energized wiring.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that covers are provided for all electrical pull boxes, junction boxes and fittings.
