29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:     a)  Trim saw operator :   On or about July 24th, 2013 the employer exposed employee(s) to noise at levels of 87.2% (equivalent to an 8 hour time weighted average TWA of  dBA) which is 1.744 times the Action Level of 50 % (equivalent to an 8 hour TWA of 85 dBA) while sorting in the production area.  Results were calculated for a 154 minute sampling period.  The employer failed to conduct audiometric testing annually.     b)  Edger saw operator :   On or about July 24th, 2013 the employer exposed employee(s) to noise at levels of 84.5 % (equivalent to an 8 hour time weighted average TWA of 88.79 db) which is 1.69 times the Action Level of 50% (equivalent to an 8 hour TWA of 85 dBA) while operating the dismantle saw.  Results were calculated for a 168 minute sampling period.  The employer failed to conduct audiometric testing annually.     c)  Hole  operator :   On or about July 24th, 2013 the employer exposed employee(s) to noise at levels of 117.5 % (equivalent to an 8 hour time weighted average TWA of 91.16 db) which is 1.175 times the Permissible Exposure Limit of 100% (equivalent to an 8 hour TWA of 90 dBA) while operating the dismantle saw.  Results were calculated for a 164 minute sampling period.  The employer failed to conduct audiometric testing annually.
