29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) Mezzanine - Boxloft - Loading/Unloading Gate Area Personal protective equipment was not provided when opening and closing the mezzanine gate exposing the employee to a 12-foot fall to the floor, as observed on or about 08/07/13. Exposure occurs prior to receipt of pallets containing unmade boxes and after the last pallet containing newly constructed boxes is lowered to the floor below.
