29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Jobsite, 501-507 South 12th Street, 3rd Floor - Employees were performing carpentry operations within 2 feet of unprotected edges 44 feet above the basement level without conventional or alternative methods of fall protection, observed on or about 6/12/13.  b) Jobsite, 501-507 South 12th Street, 3rd Floor - An employee was performing carpentry operations within 3 feet of an unprotected 54 inch x 72 inch floor hole 11 feet 2 inches above the next level without fall protection, observed on or about 6/12/13.  Paiz Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 555180, citation number 1, item number 1a and was affirmed as a final order on 11/29/12, with respect to a workplace located at 134 Catherine Street Philadelphia, PA.  Paiz Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 312494388, citation number 1, item number 1a and was affirmed as a final order on 2/23/12, with respect to a workplace located at 2015 Brown Street Philadelphia, PA.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite, 501-507 South 12th Street - Employees were performing carpentry operations within 2 feet of unprotected edges up to 44 feet above the next level without being trained in fall protection. Employees did not know the trigger height for fall protection in construction on or about 6/12/13.  Paiz Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a), which was contained in OSHA inspection number 555180, citation number 1, item number 1b and was affirmed as a final order on 11/29/12, with respect to a workplace located at 134 Catherine Street Philadelphia, PA. 29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  a) Jobsite, 501-507 South 12th Street, Unit 1 - Employees were using unprotected stairways that were not equipped with a stairrail system. This condition exposed employees to fall hazards ranging from 10 feet to 44 feet above the lower levels, observed on or about 6/12/13. Employees did not know the minimum height stairways should be equipped with stairrails on or about 6/12/13.
