29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) First Floor - Workers were exposed to a seven foot fall from the unguarded edge of an open-sided floor, on or about 06/19/2013. b) First Floor Stairway - Workers were exposed to a 10 ft. fall through a stairway opening that was not covered, on or about 06/19/2013. c) Second Floor - Carpenters whom were not provided with fall protection and exposed to a 22 foot fall from the top plate of a wood-framed exterior wall of a house while securing roof trusses in place, on or about 0/19/2013. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder: a) Front Yard - A 20 foot aluminum extension ladder used to access a low-sloped hip roof, extended only 24 in. above the ladder landing and was not secured on place or provided with a grabrail, on or about 06/19/2013.
