29 CFR 1910.106(e)(2)(ii)(b)(2): The quantity of Category 2, 3 or 4 flammable liquid located outside of an inside storage room or storage cabinet in a building or in any one fire area of a building exceeded 120 gallons: (a) On, or at dates prior to, June 19,2013, employees on the workroom floor was exposed to a fire hazard because five 55-gallon drums of flammable liquid was stored inside a building which did not have a flammable storage room. Abatement Note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.147(c)(6)(ii) The employer did not certify that periodic (annual) inspections of energy control procedures were performed. The certification should identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection. (a) In that an examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified since 2008. Specifically, reviews were not conducted to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources. Abatement Note: Abatement certification is not required for this item.
