29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)      On or about June 19, 2013, at the jobsite employees were observed working from 2-tier scaffolds approximately 14-ft high.  The working platforms were not fully planked or decked.  Employees were exposed to fall hazards.         LOYA CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(b)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313684839, CITATION NUMBER 1, ITEM NUMBER 1, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 30, 2009, WITH RESPECT TO A WORKPLACE HAZARD AT 7509 N. MAY, OKLAHOMA CITY, OK 73116.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.      On or about June 19, 2013, at the jobsite employees were observed working from 2-tier scaffolds approximately 14-ft high without guard rails provided and were not wearing personal fall arrest harness.  The employee was exposed to fall hazards.              LOYA CONSTRUCTION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.451(g)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313684839, CITATION NUMBER 1, ITEM NUMBER 4, AND WAS AFFIRMED AS A FINAL ORDER ON SEPTEMBER 30, 2009, WITH RESPECT TO A WORKPLACE HAZARD AT 7509 N. 29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:      On or about June 19, 2013, at the jobsite employees were observed working from 2-tier scaffolds approximately 14-ft high.  The scaffolds legs were laid on mud sills without using baseplates.    Employees were exposed to fall hazards.
