29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     (a)  Ground below the men on the roof - The employer did not provide head protection for the employee on the ground supporting the roofing work that happened above him.  The employee was exposed to possible head injury from debris that was tossed off of the roof into a nearby trailer, on or about June 20, 2013.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       (a)  Roof - The employer did not ensure that employees working on the roof had an approved method of fall protection.  The employees were exposed to a fall hazard of 14 feet from the roof to the ground below on or about June 20, 2013.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     (a) 167 High Street, State College, Pennsylvania - The employer did not train the employees to recognize fall hazards and OSHA required procedures for reducing fall hazards.  Four employees including the foreman worked on the roof without personal fall protection, safety nets or guardrails on or about June 20, 2013.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     (a) Center of the Building Above the Front Door - Employees used an extension ladder to access the roof from the front yard.  The top of the ladder was not three feet above the landing point and employees were exposed to a fall of 14 feet to the concrete sidewalk below on or about June 20, 2013.     (b) Right Side of Building from Ground to Balcony - Employees used a ladder to access the balcony from the front yard.  The top of the ladder was not three feet above the landing point and employees were exposed to a fall of 8 feet to the ground below on or about June 20, 2013.     (c) Right Side of Building from Balcony to Roof -Employees used a ladder to access the roof from the balcony.  The top of the ladder was not three feet above the landing point and employees were exposed to a fall of 7 feet 9 inches to the surface below on or about June 20, 2013.       Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
