29 CFR 1910.332(b)(3)(ii):  Employees were not trained in and familiar with the safety related work practices required by 29 CFR 1910.331 through 29 CFR 1910.335 that pertained to their respective job assignments specifically, the skills and techniques necessary to determine the nominal voltage of exposed live parts:    (a)    Fair Inc. dba 1 Elevator Services of Colorado at 10 E. 120th Ave, Northglenn, CO 80233:  On and before June 8, 2013, the employer did not ensure employees were trained concerning the risk of injury due to electrical shock, arc flash, or other electrical hazards associated with supply wiring in excess of 480 VAC.  In this instance the primary supply wiring for the building was accessed by an employee that did not recognize the nominal voltage of the exposed conductors inside the live front transformer located outside the hotel.  This condition exposed the employees to hazards such as shock, electrocution, arc blast, and arc flash.
