29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) On or about June 19, 2013, at the jobsite employees of a sub-contractor were observed working from 2-tier scaffolds approximately 14-ft high. The working platforms were not fully planked or decked. Employees were exposed to fall hazards of approximately 14-ft to the ground.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about June 19, 2013, at the jobsite employees of a sub-contractor were observed working from 2-tier scaffolds approximately 14-ft high without guard rails provided and were not wearing personal fall arrest harness. The employee was exposed to fall hazards.
