Worksite located at 35th and Grand, Independence, Missouri- Employees were exposed to vehicular traffic while working next to an open trench and were not protected by any means of barricading conforming to the Manual of Uniform Traffic Control Devices for Streets and Highways. 29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 35th and Grand, Independence, Missouri- Employees were exposed to a shock or electrocution hazard in that extension cord being used in the trench that was full of water had its' outer insulation breached and had been wrapped with a black tape.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:   (a). Worksite located at 35th and Grand, Independence, Missouri- Two employees were observed working in a trench in which both employees were not provided a safe means of egress from the trench. One employee located on the west end of the trench did not have access to a ladder or ramp due to the obstructions and his location. The other employee was at a depth of approx. 11-12 feet and the ladder only went down to approx. 6 feet, meaning the employee would have to climb 5-6 feet above to get to the ladder.

Worksite located at Jct. 35th and Grand, Independence, Missouri- Employees were exposed to overhead hazards from the spoil piles located on both the north and south sides of the excavation in which the the spoil and  materials were not pulled back at least 2 feet from the edge of the trench.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective:   (a). Worksite located at Jct. 35th and Grand, Independence, Missouri- An inspection had not been made by a competent person prior to employees entering the trench. Had a competent person inspected the trench, one would have noted , among other things, the lack of any means of cave-in protection, spoil piles accumulating next to the trench, unsupported walls the could fall,  proper soil classifications, and water accumulating in the trench.
