29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, warning line system and personal fall arrest system, or warning line system and safety monitoring system; or, on roofs 50 feet (15.25 m) or less in width, the use of a safety monitoring system alone:   On or about June 14, 2013, an employee was exposed to a fall hazard of approximately 18 feet to the concrete ground below, while staging material on a low slope roof, 2 and 12, without utilizing fall protection and/or safety monitor.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   On or about June 14, 2013, an employee was exposed to a fall hazard of approximately 18 feet to the concrete ground below, while accessing a roof  and using a Louisville 24 foot metal extension ladder which did not extend at least 3 feet above the landing.

29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:   On or about June 14, 2013, an employee was exposed to a fall hazard of approximately 18 feet to the concrete ground below, while accessing a roof  and using a Louisville 24 foot metal extension ladder which was set-up next to a door which was propped open.
