29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Jobsite, 501-507 Lombard Street, 3rd Floor - Workers were performing carpentry operations within 2 feet of unprotected edges 44 feet above the basement level without conventional or alternative methods of fall protection, observed on or about 6/12/13.  b) Jobsite, 501-507 Lombard Street, 3rd Floor - A worker was performing carpentry operations within 3 feet of an unprotected 54" x 72" floor hole 11 feet 2 inches above the next level without fall protection, observed on or about 6/12/13.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  a) Jobsite, 501-507 South 12th Street, Unit 1 - Workers were using unprotected stairways that were not equipped with a stairrail system. This condition exposed employees to fall hazards ranging from 10 feet to 44 feet above the lower levels, observed on or about 6/12/13.
