29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about June 18, 2013, two employees were working on a residential roof and they were not protected from falling to the ground. The fall hazard to the two employees was up to approximately 11 feet. The pitch of the porch roof was 3 in 12 and the pitch of the main roof was 4 in 12.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee: (a) Jobsite: On or about June 18, 2013, two employees who were exposed to fall hazards while working from a roof needed to be retrained on when to use fall protection.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) Jobsite: On or about June 18, 2013, there was no ladder on site for the employees to access the roof. The employees were using the mechanical material hoist as a ladder to access the roof. In addition, this mechanical material hoist had a broken "rung" that had a piece of wood pushed through it.
