29 CFR 1910.242(a): The employer did not ensure that employees were furnished with tools and equipment that were in a safe condition: a. Site - On June 20, 2013, an employee was using the Carlton (model SP7015 S/N 1J9K12122E1167287) stump grinder. This stump grinder was modified from its original design and not maintained in a safe condition. The stump grinders remote control cord was shortened and the employer had not obtained prior written approval for this modification.
