29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.    a) Oxford Mills, East Building - Three employees were performing roofing activities on a low-slope roof within one foot of an unprotected edge approximately 46 feet above ground level without conventional or alternative means fall protection.  Observed on or about 6/19/13.      b) Oxford Mills, East Building - An employee was performing roofing activities on a low slope roof within one foot of an unprotected edge, approximately 20 feet above ground level without conventional or alternative means of fall protection.  Observed on or about 7/10/13.      c) Oxford Mills, East Building - An employee was performing roofing activities on a low slope roof within approximately two feet of an unprotected edge, approximately 60 feet above ground level without conventional or alternative means of fall protection.  Observed on or about 7/10/13.      MARK KATONA ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314229154, CITATION 1 ITEM 1a, AND WAS AFFIRMED AS A FINAL ORDER ON 12/1/2010 WITH RESPECT TO A WORKPLACE LOCATED AT 2342 S. BROAD STREET, PHILADELPHIA PA 19145.      Note:  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
