On or about 05/23/2013, at the jobsite at 17 Ronnie Dr., O'Fallon, MO 63366, employees were using nail guns to install shingles on the roof of the residential structure without wearing the necessary eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about 05/23/2013, at the jobsite at 17 Ronie Drive, O'Fallon, MO 65366, employees were engaged in shingling operations without the use of any fall protection system. The employees were exposed to an approximately 11 foot fall to the ground below.
