29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: 495 Flatbush Ave, Hartford, CT 06106: On or about 6/21/2013 the employer did not provide a ladder where there was a break in elevation 36 inches from a lower roof to the pitched roof and 24 inches from the pitched roof to the main building roof. An employee was on the lower roof and he climbed the pitch roof to the main building roof.
