No ladder, ramp, or other safe means of egress were provided.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:  On or about: June 19th, 2013    Location: New Wal-Mart construction at Swansea Mall Drive, Swaansea, MA, 02777    Excavation operations were being conducted with two employees exposed to excavation hazards in a six foot deep trench. A competent person was not on site while work was being conducted. Rock and soil fissures, adjacent to employees in the trench walls, were evident.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). The employer had not complied with the provisions of 29 CFR 1926.652(b)(1)(i) in that:    On or about: June 19th, 2013    Location: Swansea Mall Dr, Swansea, MA. Walmart construction site.    Two employees were exposed to cave-in hazards while working in a six foot deep trench without a trench box or other means of protective shoring.    THE COMPANY, SITE RESOURCES LLC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.652(a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312339658, CITATION NUMBER 2, ITEM 1, ISSUED ON DECEMBER 9TH, 2008 WITH A FINAL ORDER DATE OF (DATE), AND OSHA INSPECTION NUMBER 312338452, CITATION NUMBER 1, ITEM 2, ISSUED ON SEPTEMBER 16TH, 2008 WITH A FINAL ORDER DATE OF (DATE).
