On or about 6/24/13 Location: 1 Harding Avenue, White Plains, NY 10605 a) An orange extension cord used to power a right angle abrasive wheel saw and a vaccuum cleaner was not protected by a Ground Fault Circuit Interrupter (GFCI).

The employees were not trained by the employer in the hazards related to ladder use and the procedures to minimize such hazards.
