29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection.    On or about 6/24/13    Location: 1 Harding Avenue, White Plains, NY 10605    a) An orange extension cord used to power a right angle abrasive wheel saw and a vaccuum cleaner was not protected by a Ground Fault Circuit Interrupter (GFCI).

29 CFR 1926.405(g)(2)(iii): Flexible cords were not used in continuous lengths without splice or tap:    On or about 6/24/13    Location: 1 Harding Avenue, White Plains, NY 10605     a) An orange extension cord was tapped into with wire nuts and tape approximately 3 inches from the receptical (female)end to power a temporary lighting string.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:  On or about 6/24/13  Location: 1 Harding Avenue, White Plains, NY 10605  a) Employees used a four foot aluminium "A" frame ladder that was damaged. The employees were not trained by the employer in the hazards related to ladder use and the procedures to minimize such hazards.
