29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    a. Site- On or about June 20, 2013 two employees working inside of a trench shield were not provided with safe egress in and out of the excavation. The employees were struck with flying debris from a failed pressure test of a nearby steam line and the employees had to exit the protective trench shield and climb out of the excavation.
