29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) All departments throughout the plant. On or about June 25, 2013, employer did not establish and implement lockout/tagout program for employees to receive training and utilize formal lockout/tagout procedures for energy isolation on the machines where the hazard of unexpected energization could occur.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) where hazards such as those created by the unguarded portion of the saw blade during operating cycle(s) were present:    (a) Wood shop, foam cutting unmarked vertical bandsaw No. 52ST. On or about June 25, 2013, employees were exposed to amputation and laceration hazards where unused portion of the blade was not guarded.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.213(h)(1): The sides of the lower exposed portion of the blade of radial saw(s) were not guarded to the full diameter of the blade by a device that automatically adjusted itself to the thickness of the stock and remained in contact with the material.    (a) Wood shop, DeWalt radial saw. On or about June 25, 2013 employees cross-cutting raw lumber pieces were exposed to amputation hazards where lower side guards were not affixed as designed.    There is no abatement certification or documentation required for this item.

On or about June 25, 2013, employees were exposed to entanglement hazard where a rotating horizontal shaft was exposed by the operator's station.
