29 CFR 1903.19(c)(1): The employer failed to certify to OSHA (the Agency) that each cited violation has been abated:  a)  Jobsite located at 419 5th Ave S., Nampa, ID 83651: The employer did not provide certification that Citation 1, Item 1 of inspection number 914080 was corrected.  The date by which this violation of 29 CFR 1926.501(b) (13) was to be corrected was July 23, 2013.  b)  Jobsite located at 419 5th Ave S., Nampa, ID 83651: The employer did not provide certification that Citation 2, Item 1 of inspection number 914080 was corrected.  The date by which this violation of 29 CFR 1926.503(a)(2) was to be corrected was July 29, 2013.

(a)  Jobsite located at 419 5th Ave S., Nampa, ID 83651:  The employer did not provide documentation that Citation 1, Item 1 of inspection number 914080 was corrected.  The date by which this violation 29 CFR 1926.501(b)(13) was to be corrected was July 23, 2013.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) West side of structure: On or about June 12, 2013 and times prior thereto, two employees involved in roof tear off activities on a residential structure without a means of fall protection.  Note: Abatement certification and documentation ARE required for these items.   Bruce Glenn Roofing was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 645458, citation number 1, item number 1, and was affirmed as a final order on December 11, 2012, with respect to a workplace located at 1501 W. Fort Street Bldg 2, Boise, ID.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):  a) Roof of structure; On or about June 12, 2013 and at times prior thereto, the employer did not provide adequate and/or effective fall protection training for two employees performing roof tear off activities at elevated areas of a residential structure.   Note: Abatement certification IS required for this item.
